Death Index Lookups
Washington State Death Index Pages 1961 - 1964 $4.00 These are pages from the statewide death index. They show an exact date of death, death place, age, and a death certificate number. See Sample Death Index Page and explanation of codes.
General Info: Record includes soundex code, name, gender, age, death place, death date, & death certificate number. The index is now online at LDS Family Search Labs and the Washington Digital Archives through 1960. You may also be able to find an index entry from Ancestry.com for 1965 – 1996 deaths, but they seem to have almost none from 1955 – 1964. You might also check the Washington Archives (see Links) to see if they can do a quick lookup for you. Index dates are 1961 through 1964 only.
Fees & delivery: $4.00 per lookup for a link to a digital image that you can view or print from your own computer. You will receive the page(s) showing the entry or lack of entry. Delivery time is usually 5 business days or less from the time I receive your request and payment.
